Instructions and Notes: Fill
out all four parts (I, II, III and IV).
If you are returning within
TSPA and the information is all the same, only fill I and IV.
If you are registering siblings, fill out all
three parts for the 1st person and only part I, II and IV for
the others. For example, once you push the submit button on this
form, click register another now in the top 10 things to do from
here on the next page. Please note that by registering for the
adult clinics does not guarantee that space is available. When
entering five for number of weeks, this means you will take the
same time and day every week for five weeks (i.e. five weeks Saturday
12:00-13:00 means you are registering for the next five Saturdays
from 12:00-13:00).
Required Filed's: Please fill out all the required fields marked with * such
as child's first name, last name, membership information and how you will pay for the clinic, your name,
email and you must check off to agree to the TSPA terms and conditions.
Payment Information: Payment
must be received at least 14 days in advance of the clinic. In
the case that you are registering within 14 days, please make
arrangements to pay as soon as possible to finalize registration. Please pay before the lesson for all
clinics.
